SERVICE AGREEMENT for 1:1 Coaching with Rae Anne Mullins
Terms:
Rae Anne Mullins and RAM Wellness, LLC will provide services for lifestyle coaching, nutrition coaching and/or fitness coaching for Client. This includes a plan and coaching sessions.
Length of service:
Rae Anne Mullins will continue to offer coaching services to Client based on a 1, 3 or 6 month timeframe determined by Client and Coach in the assessment call.
Payment Agreement:
Client agrees to the determined payment schedule. Client is offered monthly payments or pay in full. If Client decides to cancel coaching before said time is finished, Client is expected to continue payment until the end of agreed upon timeframe.
If Client is suffering from financial hardship, Client may discuss alternative payment methods.
Refunds:
Rae Anne Mullins will not offer refunds on completed services or plans.
Services:
Rae Anne Mullins will provide for the Client a customized plan and coaching sessions that fit the needs of the Client. Coaching calls will be held online either on the phone or on Zoom online. Sessions will be no longer than 60 minutes each. Number of coaching calls is determined by the length of service agreed upon in assessment call. This will vary from client to client, but will be stated and noted in the Plan.
Cancellation of Coaching Calls:
Client may cancel or reschedule their call with a 24 hour notice. Rae Anne has the right to charge for the service if Client does not cancel with a 24 hour notice or if Client is a No-show.
Results and Expectations:
Client results will vary. Rae Anne Mullins does not guarantee specific results. Rae Anne will to the best of her ability and knowledge provide Client with recommendations to get Client results. Client ultimately is the determinant of positive results.
Client agrees to asking for help when needed, attending all Coaching Calls, and being completely honest and transparent with Rae Anne Mullins so she can provide the best advice to Client.
Client understands that if he/she does not follow the plan or advice, they may not get results and that is not at any fault of Rae Anne Mullins.
Confidentiality:
The agreement and relationship between Rae Anne Mullins and Client is confidential. Rae Anne Mullins will not share any information about Client without Client’s consent. Rae Anne Mullins may ask for testimonials, but will not share results or information unless Client agrees.
